Below are explanations of our most important office policies and practices.

Payment

Payment is due at the time the service is provided. Please pay using cash, credit card, or debit card. We accept Visa, MasterCard, and Discover. We do not accept checks or American Express.

Appointments

Please plan to arrive promptly for your appointment. If you arrive more than 10 minutes late, as a courtesy to other patients our office has the right to reschedule your appointment.

Medication

Please bring all of your medication (or a list of medications) with you to each appointment. This will give you, the doctor, and the nurse the opportunity to discuss allergies, directions, side effects, medication interactions, and dosages. Be sure you have enough refills to last until your next appointment. Please set up your appointments to allow a two-week safety margin of medication, in case scheduling changes occur near the time of your appointment.

Insurance

As a courtesy, we will bill your primary insurance carrier if you provide us with the proper paperwork. Co-payments and deductibles are due at the time of service. Your agreement with the insurance carrier is between you and your insurance company, and as such we do not routinely research denied claims or claims not paid per contracted agreement. If your insurance carrier has not paid within 60 days from the date of billing, you will be responsible for the outstanding balance. Any services provided which are not covered under your insurance plan must be paid in full at the time of your visit or upon notice of insurance claim denial.

Cancellations

Please be respectful of other patients and the doctor by providing a 24-hour notice if you plan to cancel your appointment. If you do not provide a 24 hour notice during the weekday, a $50 “No Show or Late Cancel” fee will be charged to your account.

Personal Injury Cases & Workers Compensation

This office does not bill for auto accident, liability, lawsuit, or work-related injury. If you are treated for such, you will be responsible for payment in full at the time of service.

Late Payment

A 2% service charge may be added to any outstanding balance over 60 days old.

Delinquent Accounts

Any account not paid according to our request will be turned over to a collection agency and a 30% fee will be added to the balance. This amount must be paid in full before any further appointments can be made.

New Patient Process

Please plan to arrive 10 minutes early for your first office visit. Bring the following items with you:

  • Completed New Patient Forms (click on links below to print)
  • Insurance card
  • Picture ID, such as Driver’s License
  • All your medications (or a list), including any vitamins or supplements you take regularly
  • Name, phone number, and fax number of your previous physician
  • Immunization records
